Description

DUTIES
 
As part of California’s Coordinated Care Initiative, L.A. Care is responsible for coordinating access to Home and Community Based Services for members who need support to remain living at home. The LTSS Disability Services Specialist works within the Long Term Services and Supports Department to identify and build strong collaborative relationships with disability-related organizations throughout Los Angeles County. The Specialist serves as a liaison to disability-related organizations, works to assist each organization’s clients with access to health plan services, facilitates appropriate representation on interdisciplinary care teams, develops programs and system improvements, and collaborates with organizations to address care issues when appropriate. The Disability Services Specialist coordinates day-to-day interactions between members, case management and member services staff, and disability-related service providers. The Specialist also acts a subject matter expert on case management interdisciplinary care teams and within the Long Term Services and Supports Department. Programs developed by the Specialist will focus on the integration of LTSS services into the managed care environment and will assist the LTSS Department in developing a service delivery system that improves the well being of members while giving them an opportunity to remain living safely at home.
 
QUALIFICATIONS & REQUIREMENTS
 
EDUCATION:

Required:
Bachelor's Degree or Related Experience

Preferred:
Social Work or related Degree

EXPERIENCE:

Required:
Three to five years demonstrated experience working with people with disabilities, seniors, low economic status populations, or other diverse communities, in settings such as community-based organizations, social service agencies, clinical settings, or civic organizations.

Excellent communication, writing, grammar, editing and proofreading skills with the ability to communicate effectively with internal and external customers at all levels.

Demonstrated experience speaking to consumer groups, advocacy organizations, and other community-based organizations.

Able to work collaboratively with subject
matter experts (SMEs) and business
owners while ensuring appropriate
processes are followed to drive best
practices.

Demonstrated problem solving and creativity skills.

Effective time management, organizational and project management skills.

Proficiency with Microsoft Outlook, Word, Excel, and PowerPoint.
Ability to travel around L.A. County.

Preferred:
Experience working with Independent Living Centers and other disability-related organizations strongly preferred.

Knowledge of managed care, previous experience with a health plan, and an understanding of Medi-Cal and Medicare a plus.
