Youth Support Partner

Description

 

Youth Support Partner serves as a peer mentor, positive role model and advocate and support to youth and young adults participating in the High Fidelity Wraparound Team planning process.  Their role involves connecting with youth while working with a team in order to identify, advocate and act as a liaison between youth and other professionals and community natural supports.   Youth Support Partner must have life experience(s) in one or more of the child serving human services systems, like behavioral/mental health, intellectual disability, child welfare, juvenile justice, drug & alcohol, etc, and the ability to positively and appropriately self-disclose regarding that experience(s).   Responsibilities will include:

 

*Serving as an advocate and peer mentor for youth currently receiving services through Allegheny County’s Department of Human Services.

*Actively participating and completing the High Fidelity Wraparound Training and credentialing process and the Family Development Credential, as well as other trainings and professional development opportunities. 

*Speaking up for the youth and the Youth Support Partner role in various settings – 1 on 1; in small groups; at meetings and large conferences, etc.

*Being a resource on youth transition through the collection of information about the needs of young adults in any DHS System.

*Representing the Department of Human Services in internal and external committees, meetings, taskforces, and presentations and at community, county and state events.

       *Providing access to educational opportunities around the DHS system.

       *Coordinating youth involvement in a variety of opportunities, including but not limited to conference attendance involving out of town travel.

       *Coordinating youth involvement in evaluation projects.

*Assisting with the creation of publications, list serves or websites to promote and support related youth development activities.

*Working with teams, and particularly the Facilitators and Family Support Partners in HFW, to assist youth in identifying and coordinating services, community resources and natural supports.

*Educating youth on how to advocate for their needs; have a voice and choice in matters related to their lives.

       *Creating partnerships with youth and professionals to help meet their needs.

       *Educating youth on independence and recovery.

       *Coordinating and helping facilitate support groups and/or social events.

       *Providing outreach, recruits and identifying youth for the program.

       *Serving as a role model for youth and others.

       *Completing all required paperwork in a timely manner.

 

Excellent communication and interpersonal skills and ability to work independently, as well as part of a team are important.  Additionally individual should have a demonstrated ability to work with individuals from diverse backgrounds and be outspoken, flexible and open minded.  A valid driver’s license and access to a reliable vehicle are needed to attend meetings and make home visits to the families.

 

High School Diploma or equivalent, plus a minimum of 1 year combined work experience.  The young adult

must be an alumni from one of the Department of Human Services child serving systems(Office of Children, Youth and Families, Office of Behavioral Health, Office of Mental Retardation/Developmental Disabilities) or Juvenile Justice.
