Description

The RN, Admissions performs initial nursing intake assessment of potential hospice patients to determine the clinical appropriateness of their acceptance for service and communicates findings to the Interdisciplinary Team in an accurate and timely manner. Employee explains services of San Diego Hospice and The Institute for Palliative Medicine (SDHIPM) to patients and their families.  Employee is expected to learn and utilize a windows compliant computer system for patient charting and intra-departmental communication.  Proficiency and appropriate utilization is a requirement of the job role. 

Shift:  Position is Full Time, Four 10 hr shifts (9:00 AM to 7:30 PM), Weds-Sat or Wed-Fri, +Sun

Responsibilities:

• Performs the initial nursing intake assessment of patient service candidates, including performing physical assessment, obtaining nursing history, performing appropriate interventions under M.D. orders, making appropriate contact with M.D. as needed.
• Determines level of care in terms of location (home, skilled nursing facility, residential care facility, or hospital) and makes appropriate referrals.  Determines level of care in terms of frequency (routine care, general inpatient, or continuous care).
• Determines urgency of care needed.  Initiates appropriate intervention in urgent situations.
• Explains the philosophy and services of SDHIPM to patients and their families. Confers with other admission team members and/or SDHIPM physician to determine appropriateness for SDHIPM patient care services.
• Develops and provides a copy of the initial admission and nursing care plan relevant to the patient/family needs.  Formulates an appropriate hospice diagnosis, meeting requirements of the Medicare Conditions of Participation (CoPs).  Initiates the education of families/primary caregivers in appropriate care techniques.  Meets requirements for content and timeliness in all documentation.
• Provides admission packets to the Admission Verification Specialist in a timely manner.
• Processes outcomes by leaving detailed patient information voicemail to distribution line.
• Communicates in an effective and timely manner with team members and staff in other departments regarding the continuity of care.
• Maintains productivity standards as established by the agency.
• Provides backup for homecare patient home visits, answering incoming referral calls, review assessment packets for completeness and accuracy and performing clerical duties.
• Clinical Computer System:  Follows policies and established standards and guidelines relating to synchronization, accessing data, documentation, and equipment/data use and protection.
• Maintains confidentiality and shows professionalism, cooperation and courtesy in dealing with co-workers, supervisors and external customers.
• Performs other related duties, tasks or responsibilities as required, assigned or directed.

Education:  Must be a graduate of an approved school of professional nursing, who is currently licensed as a Registered Nurse by the State of California.  A Public/ Community/ or Home Health Nurse credential is desirable.  Must possess a valid CPR certification.

Experience/Qualifications:  Requires a minimum of two years of nursing experience in an acute hospital-based setting and a minimum of one year in a home health care setting.  Must have an understanding of the principles and practices of home care nursing, with specific knowledge of principles of nursing care within the scope of palliative medicine.  Requires knowledge and understanding of insurance coverage.  Requires a basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary health care team, and knowledge of the dying and bereavement processes.  Must possess knowledge or aptitude to operate a computer; experience with electronic medical charting software is preferred.  Current California Driver’s license and automobile insurance required for this position. Bilingual Spanish required.
